Office Policies

Appointment Cancellation Policy

If you cannot attend an appointment, we ask that you kindly provide us with at least 24 hours notice of cancellation. This makes it possible to offer your appointment time to another patient in need.

Please keep in mind, we have a 3 no show policy. Any patient who does not cancel their appointment 24+ hours in advanced will be marked as a no show. Patients who accumulate 3 no shows will be dismissed from the practice. 

Items to bring with you during each appointment:

  1. A valid ID 

  2. All updated and accurate insurance information including your insurance cards for your primary, secondary and tertiary. (Note: if you withhold any insurance information, you will be held financially liable for your medical claims. See more about this in your registration documents)

  3. A list of your current medications

  4. A method of payment for any copay, coinsurance, deductible, noncovered services, administration fees, outstanding balances.

  5. New patients should bring their medical records from their previous doctors office.

  6. Please complete the health history form which you can find on your patient portal by clicking the link above "Get In Touch".

-Please note, other departments such as medical billing, referrals, medical records, management, etc. is managed by our third party management company. They are not located in the same facility. 

Administration

Administration services are managed by a third party management organization ASL Medical Management. Our administration department handles services such as: referrals, medical records, prior authorizations, pre-certifications, complaints and grievances, contracting, and more. If you require assistance from an administration staff, please contact them via the "Get In Touch" link above. Please note, excessive services/extensive aid rendered by this third party company is the financial responsibility of the patient. Substantial administrative service fees are not covered by any medical plan and the financial responsibility of the patient.
